WHO: ROTARY MEMBERS plus household members, except less than 18 years old
WHERE: Lung Center Main T-block 2nd Floor
WHAT TO WEAR: Tops that easily exposes arm
WHAT TO BRING: Vaccination Card and ID
Who cannot come: Those with respiratory symptoms or those under quarantine or those who have not pre-registered (NO WALK-IN)

ELIGIBILITY CRITERIA FOR
SECOND BOOSTER DOSE
The DOH guidelines provide for the administration of second booster doses for individuals:
(1.) Eighteen (18) years old and above belonging to Priority Group A1: Workers in Essential Health Services, and
(2.) All individuals sixty (60) years old and above (Priority Group A2: Senior Citizens).
(3.) The SECOND BOOSTER dose must be FOUR (4) months from the FIRST BOOSTER shot. Anyone who received their FIRST BOOSTER SHOT not later than MARCH 1, 2022 may avail of the second booster jab.
ELIGIBILITY CRITERIA FOR
FIRST BOOSTER DOSE
(1.) Booster doses to vaccinated Priority Group Al or Frontline workers in health facilities
(2.) Booster/additional doses to Priority Group A2 or Senior Citizens aged 60 years old
and above and immunocompromised persons under Priority Group A3
(3.) Booster doses to vaccinated adults 12 years old and above not included in the above
mentioned categories, based on benefits and risks
